Nettleton Baptist Church cannot prevent you or your child(ren) (if applicable) from becoming exposed to, contracting, or spreading COVID-19 while attending Church for services, activities, or while using any of the church’s facilities. It is not possible to prevent the presence of the disease. Therefore, if you choose to engage in or attend any of the activities at Church and/or enter any of the Church’s premises, you may be exposing yourself to and increasing your risk of contracting or spreading COVID-19.

ASSUMPTION OF RISK: I have read and understand the above warning concerning COVID-19 for myself and for my child(ren) (if applicable). I hereby choose to accept the risk of contracting COVID-19 for myself and my children (if applicable) in order to engage, attend or participate and utilize the Church services and enter its premises. These services are of such value to me and my Child(ren) (if applicable) that I accept the risk of being exposed to, contracting, and possibly spreading COVID-19 in order to utilize the Church’s services and premises in person.

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against the Church and its officers, directors, pastor’s council, pastors, staff, elders, deacons, trustees, members, employees, volunteers or other representatives in connection with exposure, infection, and potential spread of COVID-19 related to utilizing the Church’s services and premises. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.

CHOICE OF LAW: I understand and agree that the laws of the State of Arkansas will apply to this contract and that either the Circuit Court of Craighead County or the Eastern District of Arkansas Jonesboro Division will be the exclusive jurisdiction and venue that shall apply to this contract.

I certify that I, nor anyone I have been in contact with to the best of my knowledge, has displayed any symptoms of Covid-19. By checking this box, I promise to self-isolate and take every precaution recommended by the CDC and Nettleton Baptist Church if this changes in the future. I also agree that I and my family will honor the guidelines in place for this event so as to protect the membership and guests of NBC.

Masks and other social distancing guidelines will be in place. Please be prepared to have your temperature taken upon entry.

If you have or think you might have COVID-19, it is important to stay home and away from other people. Staying away from others helps stop the spread of COVID-19. If you have an emergency warning sign (including trouble breathing), get emergency medical care immediately.
